The purpose of this study is to do a retrospective cross sectional chart review to identify how many patients having elective surgery and going through a preoperative assessment clinic have existing advance care planning (ACP)/surrogate decision making (SDM) document and determine which patients would benefit most from advance care planning prior to surgery.
