Utility of Home Health Services in Reducing Readmissions to the hospital in patients admitted with congestive heart failure exacerbation.

Summary

At the Richard L Roudebush VA hospital we adopted the transition of care for patients discharged with primary diagnosis of heart failure through implementation of the home health care. Home health care is a program that is for patients that are viewed as high risk patients due to severely reduced ejection fraction and have been admitted with congestive heart failure. These patients are then followed at home by a team. The team consisted of registered RN, LPN, pharmacist, social worker and was supervised mainly by the cardiologist. There is no data as to how this care model would work in the veteran population. We intend to evaluate the benefit of this care model in our veteran population and the cost effectiveness of this approach.
